The Challenge
The service user is usually an independent 90 year old and lives alone in a rural house away from her grown-up children. She experienced a fall which alerted family who had not been visiting due to lockdown that she was not coping well at home.
The Change
Equipment including shower chair and caddy was provided and advice about changing set up at home to encourage independence and reduce risk of falls. Community alarm, meals on wheels and enablement short-term care were sourced.
The Impact
This lady regained her independence at home. She had a positive experience with health and social care which may be important if she needs support again in future. Family living away feel reassured that her safety and wellbeing is better now.
