The Challenge
Not getting enough information to ensure hospital discharges are referred directly and quickly to our community occupational therapists, and to build contacts at a local hospital who generate the majority of our referrals.
The Change
Met the hospital occupational therapy team and spent time on the wards. Explained about our occupational therapy role and things they and we can do to help, i.e. contact us direct/highlight on the referral that needs to be seen by our team of OT's.
The Impact
Now receiving referrals from the hospital with the above information so we can see clients sooner to increase rehab potential, timely occupational therapy intervention, and reduced potential failed hospital discharges.
