Join us for an innovation café on avoiding unnecessary hospital admissions.
Occupational therapists play a critical, often under-recognised role in preventing unnecessary hospital admissions and supporting people to remain safely at home. This virtual café will explore innovative frontline examples where occupational therapy is embedded in urgent care pathways across different settings.
You’ll hear four flash presentations highlighting diverse models of OT practice focused on admission avoidance:
- Urgent Community Response with the Patient at Home: Exploring how timely OT input in the home environment prevents hospital admission
- OT and Paramedic Response Vehicle: A joint response model delivering on-the-spot assessments and interventions.
- OT in Emergency Department Rapid Assessment & Triage: Embedding OT early in the ED journey to support safe discharge.
- OT across the system: Coordinated care across sectors to proactively reduce admissions.
Session Aim:
To collaboratively identify and build a shared list of measurable outcomes and evidence-based indicators that demonstrate the impact of occupational therapy in avoiding hospital admissions.
