Supporting family to get a patient with a significant physical, cognitive and communication problems following a brain injury home from a nursing home placement.
The Change
Assessing for a specialist equipment, hands on physical rehabilitation. Spending time with the family to teach them how to maintain postures in lying.
Developing a rapport with the patient directly, during treatment sessions to promote their wishes
The Impact
Patient came home, with family and social care support. They have progressed physically, and are verbally communicating their wants, feeding themselves so no longer need a PEG.
Had a shower for the first time in 12 months.
