An exciting opportunity has arisen for a higher grade occupational therapist to join the Portsmouth Rehabilitation and Reablement Team (PRRT).
PRRT is an integrated health and social care team, providing a short term intermediate care and rehabilitation service to vulnerable and disabled service users. The team consists of occupational therapists, physiotherapists, nurses, social workers and rehab assistants. PRRT is at the forefront within Portsmouth providing urgent assessment in the community for hospital avoidance, rehabilitation and reablement at home, and facilitating a discharge to assess pathway from the acute hospital allowing patients to have their immediate needs assessed in their own home.
The post holder will provide skilled support to maximise independence and to help the individual remain living in their own home for longer.
The occupational therapy service provides intervention to patients in their own homes.
